Medzingo, LLC collects, scans and organizes medical records to assist health care professional involved in the care of its subscribers. The medical records collected include medical forms completed by its subscribers and other medical records from the subscriber, and from the subscriber's health care professionals and health care facilities obtained at the request of the subscriber or their representative.
Medzingo, LLC wishes to emphasize that medical records for the subscriber may exist of which we are unaware and/or medical records may exist which have not been forwarded to us by physicians, other health care professionals and health care facilities despite requests by the Subscriber.
Medzingo, LLC makes no claim or representation that the records included in this medical record are all of the medical records that exist for this Subscriber.
